Assembling Tax Information

Guide to Assembling Personal Tax

Please use the following document list as a guide for assembling your personal tax information.

Note that the list is not all inclusive, and some items may not pertain to you. However, it is your responsibility to make certain the information provided to us is complete.

  • All W-2 forms for wages, salaries, and tips;
  • All 1099 forms for interest (1099-Int), dividends (1099-Div), retirement distributions (1099-R), social security benefits, disability, unemployment compensation, gambling winnings (W-2G), miscellaneous income (1099-Misc), stock sales (1099-B), etc.;
  • All income information for children, if you want us to prepare any required returns;
  • All rental income and expenses;
  • Cost basis on sold investments (This must be provided by the client if not accurately and completely reported by the brokerage firm. Generally only full service brokerage firms provide cost basis);
  • Schedule K-1's showing income from partnerships, S-Corporations, estates, and trusts (Only if not prepared by this office);
  • Self employment health insurance;
  • 1098 statements supporting deductions for mortgage interest on your home or home equity loans and real estate taxes paid;
  • Total of charitable contributions, and details for any non-cash contributions over $500;
  • Any other itemized deductions, for example, medical, dental and prescription drugs expenses, major purchases for sales tax deductions, unreimbursed employee expenses;
  • Any other miscellaneous information, for example, HSA contribution/distributions, alimony payments, student loan interest, teachers expense;
  • Any tax notices sent to you by the IRS or other taxing authority (Only if you have not already sent to this office);
  • A copy of your tax returns (Federal & State) from last year, (only if not prepared by this office);
  • Final closing/settlement statements regarding any purchase, sale, or refinance of real estate. If you sold real estate, we need to know the date of acquisition and cost basis, including improvements.

  • Year-end bank statement for all bank accounts. Include bank reconciliation, if available;
  • Revenue and expense information (categorized) for the period;
  • If using QuickBooks, Peachtree or any other accounting package, we will need a copy of your backup (CD or e-mail). Please include the password, if applicable;
  • Copy of all payroll tax returns - Forms 941, UCT-6, and 940 (Only if not prepared by this office);
  • Copy of W-2 & W-3 forms (Only if not prepared by this office);
  • Year-end balance of all loans payable and notes receivable;
  • All new loan closing documents or financing agreements for the current year;
  • Inventory balance at year-end, if applicable;
  • Any tax notices sent to you by the IRS or other taxing authority (Only if you have not already sent to this office);
  • A copy of your tax returns (Federal and State) from last year. (Only if not prepared by this office);
  • A detailed list of fixed assets purchased during the year with the date, description and cost;
  • A detailed list of fixed assets sold, traded or abandoned (with dates)
